Post-acute care encompasses a range of medical and health services that support a patient’s ongoing recovery from a recent illness, injury, or hospital visit.
Goals of Post-Acute Care
The goals of post-acute care are far-ranging and will be dependent on patient needs. In essence, post-acute care supports recovery after a hospital stay and functional recovery of older patients to prevent unnecessary hospital readmission. Treatment methods can include additional therapies such as physical or occupational therapy.
Appropriate post-acute care also aims to avoid premature admission to long-term care facilities through palliative, comfort, or end-of-life care.
